Liability Waiver and Release of Claims

I, [Client's Full Name], hereby voluntarily participate in the coaching or workshop event titled "[Event Name]" (the "Event") organized and conducted by Jennie Queen, hereinafter referred to as the "Coach."

In consideration for my participation in the Event, I understand and agree to the following:

  1. Assumption of Risk: I acknowledge that the Event may involve activities that could be physically or mentally demanding. I understand that there are inherent risks associated with such activities, and I voluntarily choose to participate, knowing that I assume these risks.

  2. Health and Safety: I affirm that I am in good physical and mental health to participate in the Event. If I have any medical conditions, allergies, or other health-related concerns, I have informed the Coach in writing prior to the Event.

  3. Responsibility for Actions: I agree to take full responsibility for my actions during the Event and to act in a manner that does not endanger myself or others. I will follow all safety guidelines and instructions provided by the Coach.

  4. Release of Claims: I, on behalf of myself, my heirs, assigns, and personal representatives, hereby release and discharge the Coach from any and all claims, demands, liabilities, actions, or causes of action, whether at law or in equity, arising out of my participation in the Event.

  5. Indemnification: I agree to indemnify and hold harmless the Coach from any and all claims or actions brought by third parties as a result of my participation in the Event.

  6. Photographic and Video Release: I grant permission to the Coach to take photographs and videos of me during the Event. I understand that these images may be used for promotional or educational purposes without compensation to me.

  7. Right to Refuse: The Coach reserves the right to refuse participation or remove any participant from the Event who displays disruptive or unsafe behavior, as determined by the Coach's sole discretion.

I have carefully read and understood this Liability Waiver and Release of Claims, and I voluntarily agree to its terms. I acknowledge that I have had the opportunity to seek legal counsel if desired.
